Discharge & Long term care

Before your discharge, we will educate you, your caregiver, and your family about managing your medications, taking care of your surgical incision, and work with you to schedule follow up visits. 

Most medications will be required immediately after transplant surgery. Over the first year, we will adjust the medications including lowering the dosage and frequency of some of them and possibly discontinuing a few of them.

After your hospital discharge, we will monitor you closely on an outpatient basis. Our transplant team will schedule regular tests, including blood work, ECGs, heart biopsies and clinic visits with a transplant cardiologist. Our transplant team will communicate with your cardiologist and primary care physician to provide seamless, coordinated care.

Going Back to Work

The appropriate time for you to return to work will depend on at least two factors: how you are feeling after surgery, and what type of work you will be doing. As soon as your incision heals, and you feel better, you can discuss returning to work with your cardiologist. Some patients return quickly, while others take more time. Typically, patients manage to return to work within 6 months after surgery.
